- We know your time is valuable. To help us run on time, please arrive 20 minutes before your scheduled appointment time for registration.
- Please bring a valid photo identification (driver’s license) and insurance card, as you will not be able to be seen without them.
- If your insurance has changed, please let us know before your appointment day, as we verify all insurance prior to the appointment.
- A parent or legal guardian must accompany any minor (under 18 years old) for an appointment.
